Define what each vendor type must provide — licenses, insurance, nonprofit status, permits. Vendors are asked for exactly this on the registration form and in their portal, the paperwork stays on their account for future events, and the expiry dates you ask for here drive the compliance status on every vendor record.
MDARD food license — Mobile Food Establishment, Transitory Food Unit or Temporary Food Establishment

Every food truck needs a current license from MDARD or the delegated local health department. A Mobile Food Establishment returns to a licensed commissary every 24 hours; a Transitory Food Unit may operate statewide without one; a Temporary Food Establishment covers a single event of up to 14 days. Licenses renew annually — Genesee County's licensing year runs May 1 through April 30.

A Mobile Food Establishment that is not fully self-contained must have a written agreement with a licensed commissary kitchen, and the commissary verification form must be carried in the unit. Not required for a Transitory Food Unit.
Certified Food Protection Manager (ANSI-accredited, e.g. ServSafe)

Michigan's Food Code requires every food operation to employ at least one ANSI-accredited certified food protection manager, and the certificate must be available at the unit. Certificates renew every five years.
General Liability Insurance — $1M, City of Davison as additional insured

Certificate of general liability insurance, $1M per occurrence, naming the City of Davison as an additional insured. Kept on file and checked for the expiry date.
Michigan Sales Tax License (seller's permit)

Prepared and packaged food sold at retail is taxable in Michigan at 6%, so food vendors need a current sales tax license. Unprepared food for home consumption is exempt.
State-issued photo ID for everyone working the unit
